Greater continuity of care for Our Medical patients with MyMedicare
MyMedicare is a new voluntary patient registration model aimed at strengthening the relationship between patients, their general practice, general practitioners, and primary healthcare teams.
Patients register with MyMedicare and select their general practice, for instance Our Medical, and at the same time they can also nominate a preferred GP from the practice.
This registration will let your local Our Medical centre know that you see them as your regular practice and allow them to provide better coordinated and continuous healthcare.
MyMedicare Patient Benefits
MyMedicare patients will receive greater continuity of care with their registered practice, thereby improving health outcomes. In the future, patients who live in residential aged care homes will benefit from more regular GP visits and better care planning, and patients who frequently visit hospital will get connections to more appropriate care in general practice.
Registering with Our Medical will make it easier to receive tailored services that are suited to your needs as a patient.
Eligibility Requirements
To be eligible for MyMedicare you must have had 2 visits in the past 24 months with the Our Medical centre you would like to register with, and you must have either a Medicare card or Department of Veterans’ Affairs (DVA) Veteran Card.
Exemptions from all eligibility requirements will apply to people who are facing hardship including people experiencing domestic and family violence and/or homelessness. A trusted practice or GP will be able to help with direct registration in MyMedicare.
How to Register
Patients can register for MyMedicare from 1 October 2023.
Register by logging into your Medicare online services - Medicare Online Account or via the ‘Express Plus Medicare’ mobile app.
When registering, simply enter the centre name for your local Our Medical centre, these will be listed as:
